What was observed

The line [agent] review: joshtronic/igor#N review call failed (attempt N) recurred 19 times on 2026-07-28. Sampled occurrences span 13:17:17 to 15:01:50, emitted by tick.sh under different PIDs on each tick. The failures are not confined to a first attempt: an (attempt 2) line appears at 14:08:49 for the same PR (#449) that failed at (attempt 1) at 13:17:17, and a fresh (attempt 1) for #449 appears again at 15:01:50. No corresponding success line for the review call appears in the retained signatures.
